However, many dental practices still employ amalgam if it is clinically indicated. Even if some <a href=\"https:\/\/www.initial.co.uk\/dental-practices\/\">dentists<\/a> do not personally use the material, they may still encounter patients with amalgam restorations in place. Thus, it is vital that dental professionals know how to safely and effectively dispose of their amalgam waste.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Mercury in the environment<\/h2>\n\n\n\n<p>Mercury can enter and contaminate the environment through both natural and anthropogenic processes.<\/p>\n\n\n\n<p>The most common natural sources of mercury in the environment include emissions from volcanoes, degassing from the Earth\u2019s crust and evaporation from water bodies. Man-made activities including mining, burning fossil fuels and certain waste releases elemental mercury into the air. Mercury present in soil can be due to fertilisers, fungicides and from solid waste.<sup><a href=\"https:\/\/www.who.int\/news-room\/fact-sheets\/detail\/mercury-and-health#:~:text=Health%20effects%20of%20mercury%20exposure&amp;text=The%20inhalation%20of%20mercury%20vapour\">1<\/a><\/sup><\/p>\n\n\n\n<p>Mercury\u2019s effects on health, the environment and wildlife have been extensively researched, and it is considered by the World Health Organization (WHO) as one of the top ten chemicals, or groups of chemicals, that are a major public health concern.<sup>ii<\/sup> Once in marine environments, it is transformed into methylmercury by bacteria. This can then bioaccumulate in fish and shellfish. Larger predatory fish may also have high levels of mercury, due to their consumption of organisms that contain the substance, such as plankton.<sup>ii<\/sup> Human consumption of fish is one of the main causes of mercury exposure in humans.<sup><a href=\"https:\/\/www.epa.gov\/mercury\/how-people-are-exposed-mercury#:~:text=The%20most%20common%20way%20people\">2<\/a><\/sup><\/p>\n\n\n\n<figure class=\"wp-block-image size-large is-style-default\"><img loading=\"lazy\" decoding=\"async\" width=\"900\" height=\"900\" src=\"https:\/\/www.initial.co.uk\/blog\/wp-content\/uploads\/2022\/03\/Large-Blog-Image-1.jpg\" alt=\"Fish and shellfish bioaccumulating methylmercury in a coral reef \" class=\"wp-image-2239\" srcset=\"https:\/\/www.initial.co.uk\/blog\/wp-content\/uploads\/2022\/03\/Large-Blog-Image-1.jpg 900w, https:\/\/www.initial.co.uk\/blog\/wp-content\/uploads\/2022\/03\/Large-Blog-Image-1-300x300.jpg 300w, https:\/\/www.initial.co.uk\/blog\/wp-content\/uploads\/2022\/03\/Large-Blog-Image-1-150x150.jpg 150w, https:\/\/www.initial.co.uk\/blog\/wp-content\/uploads\/2022\/03\/Large-Blog-Image-1-768x768.jpg 768w, https:\/\/www.initial.co.uk\/blog\/wp-content\/uploads\/2022\/03\/Large-Blog-Image-1-55x55.jpg 55w\" sizes=\"auto, (max-width: 900px) 100vw, 900px\" \/><figcaption class=\"wp-element-caption\">Methylmercury can bioaccumulate in fish and shellfish, as well as their feeding predators.<\/figcaption><\/figure>\n\n\n\n<h3 class=\"wp-block-heading\">Amalgam in dentistry<\/h3>\n\n\n\n<p>Dental amalgam was historically a popular filling material, as it was relatively cost-effective, durable and easy to place.<sup><a href=\"https:\/\/jada.ada.org\/article\/S0002-8177(19)30601-4\/fulltext\">3<\/a><\/sup> However, with the increased awareness of its impact on health and the environment,<sup><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/7734058\/\">4<\/a><\/sup> measures to phase out amalgam\u2019s use have been proposed.<sup><a href=\"https:\/\/www.mercuryconvention.org\/en\/node\/252\">5<\/a><\/sup> Minamata disease, first identified in Japan in 1956, was the result of the release of methylmercury in the industrial wastewater from a chemical factory, continuing from 1932 to 1968. The Minamata Convention, named in honour of the city and the events that occurred, formed in 2013 to protect the health of humans and the environment from man-made emissions and the release of mercury.<\/p>\n\n\n\n<p>The European Commission released its Regulation on Mercury in 2017, which did not ban the use of amalgam in dentistry, however, it did prohibit its use for dental treatment of \u201cdeciduous teeth, children under 15 years and of pregnant and breastfeeding women, unless deemed strictly necessary by the dental practitioner on the ground of specific medical needs of the patient\u201d.<sup><a href=\"https:\/\/ec.europa.eu\/environment\/chemicals\/mercury\/regulation_en.htm#:~:text=Regulation%20(EU)%202017%2F852\">6<\/a><\/sup><\/p>\n\n\n\n<p>The European Commission also launched a study on the \u201cAssessment of the feasibility of phasing out dental amalgam\u201d, and they found that between the last assessment of dental amalgam from 2010 and when the study was published (2020), amalgam use dropped by an estimated 43%.<sup>i<\/sup> Nonetheless, the British Dental Association (BDA) argues that amalgam remains well established and a desirable option in certain cases.<sup><a href=\"https:\/\/bda.org\/news-centre\/latest-news-articles\/Pages\/Vital-UK-dentistry-avoids-a-rapid-phase-out-of-dental-amalgam.aspx\">7<\/a><\/sup> <\/p>\n\n\n\n<p>The BDA states that there is a lack of alternative materials that provide equivalent advantages, other restorative materials may trigger negative environmental outcomes (although they note that more research is needed), and that dentists need to make individual decisions and assessments regarding the clinical situation, with the best interests of the patient at heart.<\/p>\n\n\n\n<figure class=\"wp-block-image size-large is-style-default\"><img loading=\"lazy\" decoding=\"async\" width=\"900\" height=\"900\" src=\"https:\/\/www.initial.co.uk\/blog\/wp-content\/uploads\/2022\/03\/Large-Blog-Image-2.jpg\" alt=\"Amalgam use on teeth gold silver white\" class=\"wp-image-2240\" srcset=\"https:\/\/www.initial.co.uk\/blog\/wp-content\/uploads\/2022\/03\/Large-Blog-Image-2.jpg 900w, https:\/\/www.initial.co.uk\/blog\/wp-content\/uploads\/2022\/03\/Large-Blog-Image-2-300x300.jpg 300w, https:\/\/www.initial.co.uk\/blog\/wp-content\/uploads\/2022\/03\/Large-Blog-Image-2-150x150.jpg 150w, https:\/\/www.initial.co.uk\/blog\/wp-content\/uploads\/2022\/03\/Large-Blog-Image-2-768x768.jpg 768w, https:\/\/www.initial.co.uk\/blog\/wp-content\/uploads\/2022\/03\/Large-Blog-Image-2-55x55.jpg 55w\" sizes=\"auto, (max-width: 900px) 100vw, 900px\" \/><figcaption class=\"wp-element-caption\">Amalgam use is still considered by the BDA as a preferable material against the alternatives.<\/figcaption><\/figure>\n\n\n\n<h3 class=\"wp-block-heading\">Things to consider<\/h3>\n\n\n\n<p>Despite its apparent decreasing usage, dental practices that do handle amalgam must continue to segregate and dispose of the material to the highest standard, ensuring compliance with amalgam disposal regulations.<\/p>\n\n\n\n<p>The Regulation on Mercury stated that, as of 1 January 2019, dental facilities handling dental amalgam, whether that be using amalgam and\/or extracting dental amalgam fillings, must have an amalgam separator to ensure the prevention of its release into wastewater systems.
